OrthoPaid Terms & Conditions
Grace Period:
1) OrthoPaid provides a 14-day grace period for delinquent accounts. During this time, the customer’s access to the OrthoPaid providers services remains active.
2) Notification: On the 15th day after delinquency, OrthoPaid will notify the orthodontic provider that the grace period has expired, allowing the provider to enforce the legally binding terms & conditions of the contract. The provider will no longer be the customers doctor per the legally binding OrthoPaid contract, and the provider will assume the customer is transferring elsewhere.
3) Reinstatement Fee: To be seen by the OrthoPaid provider again, delinquent customers must pay a $99 reinstatement fee on the OrthoPaid website. A new credit card or checking account, unrelated to the provider’s office, is required during this process.
4) Notification to Provider: Once the reinstatement fee is received, OrthoPaid will promptly notify the provider of the customer’s choice to continue.
5) Treatment Option Selection: Customers will indicate on the OrthoPaid website their preferred treatment option from the contract.
6) Mediation: OrthoPaid will mediate between the customer and the provider to ensure that the delinquency issue is addressed and does not recur.
7) By utilizing OrthoPaid services, customers agree to abide by these terms and conditions. Failure to comply may result in further actions as outlined in the OrthoPaid contract.
